You know it. We know it. And thanks to UCP Central PA (UCP), individuals of diverse abilities across the midstate have come to know it too: that sense of comfort in having a safe, secure, and nurturing place to call home.

UCP’s Residential Services offer a variety of options to meet the unique needs of each individual. Options include:

Licensed Residential Services

Participants live in one of UCP’s owned or leased single family homes with 24/7 staffing support. The homes support 1-4 adults, offering individualized life skills instruction to increase independence. Our goal is for every person we support to be in a position to live as independently as they are able, and to enjoy a productive, inclusive, meaningful, and fulfilling life.

Supported living

Participants live in and hold the lease on their own apartments, with 24-hour access to direct and remote staff and services, as needed. This flexible, customized support is designed to help individuals maximize their independence. The focus is on building skills for independent living, including home and money management, health and wellness, and community engagement. Additional services may include assistance for gainful employment and opportunities to participate fully in community life.

Lifesharing

This model is exactly what it sounds like: a host family welcomes an individual with disabilities into their home to share daily life with them. In addition to offering a safe and nurturing place to call home, the family also provides 24-hour direct and indirect services and supports for the individual as needed. The host family works with the individual on life skills and discovering new interests while the individual grows into the rhythms and routines of everyday family life.

Pathways Academy

This innovative residential option provides transitional housing and curriculum-based support for individuals who have both the desire and the ability to live independently. Academy residents spend anywhere from 9-24 months developing the skills and resources needed to transition into independent living in the community with minimal natural supports. To date, Pathways Academy boasts 20 graduates who have gone on to live on their own (or with a roommate) in the housing and community of their choosing. Another four individuals are midway through the Academy curriculum.

Respite

Participants are given the opportunity to have a short-term stay (a few days to a couple of weeks) to offer relief to the caregiver or in the event of emergency circumstances. Care is tailored to the individual, with UCP staff providing 24-hour support. During their time in respite, participants have opportunities to work on relationship-building skills and community integration.

Our staff develop close bonds with the individuals as they encourage them to choose the housing and the lifestyle that will empower them to live as independently as possible.

The steadfast commitment of UCP’s Residential Team to person-centered care and operational excellence has earned the agency Select Tier Provider status under Performance-Based Contracting through the Pennsylvania Office of Developmental Programs (ODP). Achieving Select status is a significant accomplishment that distinguishes UCP among residential providers across Pennsylvania.
